1. Introduction

Sjogren’s syndrome (pronounced SHOW-grins) is a common autoimmune disease characterized by dry eyes and dry mouth. This condition is often associated with other autoimmune diseases such as rheumatoid arthritis and systemic lupus erythematosus. The pathophysiology primarily involves the immune system abnormally attacking the body’s exocrine glands, leading to a decrease in the secretion of tears and saliva. This case report aims to discuss a patient whose symptoms improved significantly after 3 months of Traditional Chinese Medicine (TCM) treatment, with improvements also reflected in blood test markers.

2. Overview of Sjogren’s Syndrome

Primary Symptoms: Dry eyes: burning sensation, foreign body sensation, itching Dry mouth: difficulty swallowing and speaking, cotton mouth

Associated Symptoms: Joint pain, swelling, and stiffness Swelling of the salivary glands Dry skin or rashes Vaginal dryness Persistent dry cough Chronic fatigue

Pathogenesis: This disease is an autoimmune disorder that can affect the joints, thyroid, kidneys, liver, lungs, skin, and nervous system. Genetic susceptibility and environmental infections (viruses or bacteria) are considered major triggers.

3. Case Data

Patient Background: Female, 40 years old, Married with children. First Consultation Date: November 4, 2024 Diagnosis: Diagnosed with Sjogren’s syndrome by a rheumatology specialist Main Symptoms: Fatigue, hair loss, dry mouth, vaginal dryness, cheek redness Past Medical History: Uterine fibroids (4.6 cm)

Blood Test Results: 

Western Medicine Treatment Plan: Hydroxychloroquine was recommended, but the patient could not tolerate the side effects and turned to TCM treatment.

4. TCM Diagnosis and Treatment Plan

TCM Diagnosis: Pulse: Deep, replete, moderate Syndrome Differentiation: Kidney yin vacuity, liver yin vacuity, lung yin vacuity, leading to insufficient body fluids and internal dryness caused by yin deficiency and internal heat.

Treatment Principles: Nourish yin, supplement qi, nourish blood, and engender liquids. Clear vacuity heat, moisten dryness, and allay thirst.

5. TCM Treatment Plan

Prescription Composition: Base Formula: Pulse-Engendering Beverage (Shēng Mài Yǐn) 5g, Lophatherum and Gypsum Decoction (Zhú Yè Shí Gāo Tāng) 5g, Six-Ingredient Rehmannia Pill (Liù Wèi Dì Huáng Wán) 5g Single Herbs Added: Lycium berries (gǒu qǐ zǐ) 2g, Lily bulbs (bǎi hé) 2g, Dendrobium (shí hú) 2g, Glehnia root (běi shā shēn) 2g, Solomon’s seal root (yù zhú) 2g, Silk tree flower (hé huān huā) 1g, Platycodon root (jié gěng) 2g, Polygonatum root (huáng jīng) 1g.

Herbal Formula Interpretation: Pulse-Engendering Beverage (Shēng Mài Yǐn): Supplements Qi and nourishes yin, engenders liquids and constrains sweat, restores the pulse, and clears vacuity heat. Lophatherum and Gypsum Decoction (Zhú Yè Shí Gāo Tāng): Clears heat, engenders liquids, supplements qi and harmonizes the stomach, improving dry mouth due to damage to liquids. Six-Ingredient Rehmannia Pill (Liù Wèi Dì Huáng Wán): Nourishes the liver and kidneys, regulates yin and yang, and suppresses upflaming vacuity fire.

6. Treatment Process and Results

After taking the formula continuously for 3 months (until February 4, 2025), the patient showed the following improvements:

  1. Blood Test: ANA changed from strongly positive to weakly positive
  2. ESR: Improved from 32 mm/h to normal 14 mm/h
  3. Uterine Fibroids: Completely disappeared, confirmed by gynecological ultrasound
  4. Hair Loss: Improved, with new hair growth
  5. Energy Levels: Increased, no longer dependent on coffee for energy
  6. Dry Mouth: Significant relief of symptoms The patient expressed a high level of satisfaction with the treatment outcomes.

7. Discussion

This case illustrates that when TCM is applied reasonably using pattern differentiation as the basis for determining treatment, it can effectively improve symptoms and systemic issues related to autoimmune diseases like Sjogren’s syndrome. By nourishing yin, engendering liquids, and clearing vacuity heat, the treatment helped restore the body’s internal environment and promote overall systemic improvement. Additionally, this case suggests that TCM might play a role in managing secondary complications, such as uterine fibroids.

8. Summary

Autoimmune diseases are not necessarily incurable or uncontrollable. Autoimmune diseases such as rheumatoid arthritis, eczema, psoriasis, and lupus can be managed effectively by controlling and reducing the immune system’s attack on the body. TCM offers another important adjunctive therapy, while Western medicine’s blood tests provide crucial insights and confirmation of post-treatment results, helping in the management of autoimmune diseases like Sjogren’s syndrome. The integration of both TCM and Western medicine brings the greatest benefits to patients.

9. Conclusion

Traditional Chinese Medicine provides a viable adjunctive therapy to aid in the management of autoimmune diseases like Sjogren’s syndrome. Further clinical research and controlled trials are needed to validate these findings.

一、前言

修格蘭氏症候群(Sjogren’s syndrome)是一種常見的自身免疫性疾病,以眼乾、口乾為主要特徵。此病常與其他免疫性疾病並存,如類風濕性關節炎、系統性紅斑狼瘡等。病理機制主要為免疫系統異常攻擊體內泌液腺體,導致淚液與唾液分泌減少。 本病例分享旨在介紹一例患者在使用中藥治療三個月後,症狀顯著改善並伴隨血液檢查指標好轉的案例。

二、乾燥綜合症概述

主要症狀: 眼乾:燒灼感、異物感、瘙癢; 口乾:吞嚥、言語困難,口感似棉絮。

可伴隨症狀: 關節疼痛、腫脹與僵硬; 唾液腺腫大; 皮膚乾燥或皮疹; 陰道乾澀; 持續性乾咳; 長期疲勞。

發病機制: 本病為自身免疫性疾病,可影響到關節、甲狀腺、腎臟、肝臟、肺部、皮膚與神經系統。遺傳易感性與環境感染(病毒或細菌)被認為是主要誘因。

三、病例資料

患者背景: 女性,40歲;已婚,有孩子;首次就診2024年11月4日; 診斷:由風濕科專科醫師確診為乾燥綜合症; 主要症狀:疲勞、脫髮、口乾、陰道乾澀、顴部面部紅斑; 既往病史:子宮肌瘤(4.6公分)。

血液檢查結果:

西醫專科治療方案: 建議使用羥氯奎寧(Hydroxychloroquine),但患者因無法耐受副作用而轉而尋求中醫治療。

四、中醫辨證施治

中醫診斷: 脈象:沉、實、適中; 辨證:腎陰、肝陰、肺陰虛損導致津液不足,陰虛內熱生內燥。

治療原則: 滋陰補氣,養血生津;清虛熱,潤燥止渴。

五、中藥治療方案

處方組成: 基礎方: 生脈飲(生脈飲)5克, 竹葉石膏湯(竹葉石膏湯)5克, 六味地黃丸(六味地黃湯)5克加單味藥: 枸杞子2克, 百合2克, 石斛2克, 北沙參2克, 玉竹2克, 合歡花1克, 桔梗2克, 黃精1克

方劑解讀: 生脈飲:益氣養陰,生津斂汗,補氣復脈,清除虛熱; 竹葉石膏湯:清熱生津,益氣和胃,改善津傷口乾; 六味地黃丸:滋養肝腎,調和陰陽,抑制虛火上炎。

六、治療過程與效果

患者在連續服藥三個月後(至2025年2月4日)總結如下改善情況:

  1. 血液檢查:ANA由強陽性轉為弱陽性;
  2. ESR由32mm/h恢復至正常14mm/h;
  3. 子宮肌瘤完全消失,經婦科專科超音波確認;
  4. 脫髮改善,新髮增生;
  5. 精力充沛,無需依賴咖啡提神;
  6. 口乾症狀明顯緩解。 患者對療效表示高度滿意。

七、討論

本案例顯示,當中醫藥在辨證論治的基礎上合理實施時,能夠有效地改善如乾燥症等自體免疫疾病的症狀與系統性問題。透過滋陰、生津與清除虛熱的治療原則,得以調整體內環境,促進整體系統的改善。此外,本案例亦顯示中醫可能在處理如子宮肌瘤等次發性併發症方面扮演一定角色。

八、摘要

自體免疫疾病 非一種不治/控之症,自體免疫疾病例如 風濕 , 濕疹 , 牛皮癬, 紅斑狼瘡等, 只要能調節及減低自身免疫系統攻擊自身 ,病者可以與自體免疫疾病和諧生活。 中醫提供了另一種重要的輔助療法,而西醫的血液檢測亦提供了非常重要的線索及治療後結果的肯定,有助於自體免疫疾病(如乾燥症)的管理。中西醫結合治療使病者最大的受益。

九、結論

中醫提供了一種可行的輔助療法,有助於自體免疫疾病(如乾燥症)的管理。仍需進一步的臨床研究與對照試驗,以驗證上述發現。
